To be effective, notifications must include the following information:

(i) a physical or electronic signature of a person authorized to act on behalf of the owner of the copyright that has been allegedly breached;

(ii) identification of works or materials being breached;

(iii) identification of the content that is claim to be infringing and or breaching including, information as to that location of the content that the copyright owner seeks to have detached, with satisfactory detail so that the visitor is proficient of finding and corroborating its existence;

(iv) contact information about the announce party including address, telephone number and email address;

(v) an affidavit that the assert party has a good faith belief that the content is not authorized by the copyright owner, its agent, or the law;

(vi) an affidavit made under penalty of perjury that the information provided is accurate and the notifying party is authorized to make the complaint on behalf of the copyright owner.
